Event Questionnaire

Please fill out the information below (if you have already booked your band with us), including as many details as possible. Our Event Coordinator will follow up to confirm the timing and other specifics a few weeks before your event. Please submit your Song List separately, using our website form.

  • Please describe the reception location, such as under tent, in boathouse, private home, hotel ballroom, etc.
  • One 20-amp circuit is required for ceremony or cocktail musicians, unless the performance is acoustic.
  • Two dedicated 20-amp circuits are required for the reception band, with electricity run to the stage before the band arrives. The band does not travel with extension cords that go beyond the general stage area.
  • Where will the band be unloading their equipment? Please share any special directions or instructions from your venue.
